How to file a claim for covered services
Submit your claims electronically
Quickest and fastest way to get your claims reimbursement
Register for the AFSPA Member Portal and complete our Electronic Fund Transfer (EFT) form. Upload your claim, receipt and other supporting documents via the portal for the fastest processing. Complete the EFT form to get your claims reimbursement into your U.S. bank account right away. Read our FAQs.
Important things to know about EFT:
- Only one bank account per family is permitted
- The EFT Authorization Form only allows the Plan to deposit funds into your bank account. The Plan cannot retrieve funds from your bank account.
- The Plan does not charge a fee for EFT service, but your bank may charge a small transaction fee.

How to submit a claim from a U.S. Provider
- Patient’s name, date of birth, address, phone number and relationship to enrollee
- Patient’s Plan identification number
- Name, address, and tax identification number of the person or company providing the services or supplies. (Tax ID number not required for providers outside the U.S.)
- Dates that services or supplies were furnished
- Diagnosis
- Type of each service or supply
- Charge for each service or supply
- Valid medical or ADA dental code (not required for overseas claims) or description of each service or supply

How to submit a claim from an Overseas Provider
FSBP pays for claims for providers outside the U.S. at the in-network co-insurance benefit. When filing a claim for services rendered by an overseas provider, bills and receipts should be itemized and show:
- Patient’s name, date of birth, address, phone number and relationship to enrollee
- Patient’s Plan identification number
- Full name and address of the provider, including city, postal code and country
- Dates that services or supplies were furnished
- Diagnosis/reason for visit Overseas Claims (include a description of the sickness or accident)
- Type of each service or supply
- Charge for each service or supply

Mail to/from overseas significantly increases the time to receive claims reimbursement
Massage Therapy Claims - U.S. and Overseas
FSBP covers 50 massages, 50 chiropractic, and 50 acupuncture visits per calendar year per member. You must pay upfront for the services and submit an itemized bill for reimbursement. We cover up to $75 per visit per service.
Members in the U.S. can visit any therapist, but they must be licensed. The itemized bill must include the provider’s Massage Therapy Licensed Number (if a United States provider and applicable per state law).
The following information is needed on an itemized bill for reimbursement:
- Patient’s name
- Patient’s date of birth
- Patient’s ID number
- Provider/massage therapist name and business address
- License or certification number (if a United States provider and applicable per state law)
- Date of each service
- Description of service rendered
- Cost for each service
- Copy of your paid receipt

Follow the Federal Employees Health Benefits Program disputed claims process outlined in Section 8 of the FSBP Plan Brochure if you disagree with our decision on your claim.
