This form is intended solely for healthcare providers that do not have direct billing arrangements with AFSPA. It is for submitting bills and invoices directly to AFSPA for services provided to members of the Foreign Service Benefit Plan.

Combine the member’s bill or invoice with other relevant documents into a single file to prevent delays in claim processing. The maximum file size allowed is 50 MB.
