Simple Step 1: Take a Health Risk Assessment

The first Simple Step is to assess your current health with a Health Risk Assessment (HRA). You can do this through an easy questionnaire in My Online Services (MOS) that asks you for basic health information similar to what you might be asked at an annual exam. Or, you can speak directly with a trained health coach to complete the assessment by telephone.  Simply call 1-866-237-1442. The online version will take about 20 minutes, while the phone conversation takes about 15 minutes. 

Completing the assessment helps you – and us – better understand what steps you need to take to be healthier.

And, you earn a $100 incentive reward which will be deposited in a Wellness Incentive Fund Account to reimburse you for certain unreimbursed medical expenses. The reward can be used to offset your calendar year deductible, coinsurance, prescription copays, non-covered dental and vision expenses and other certain unreimbursed medical expenses ("Eligible Medical Expenses") as defined by Internal Revenue Code Section 213(d). You can earn this reward once per calendar year.*

Simple Step 2: Complete a Biometric Screening

The second Simple Step in our wellness program asks you to take action offline: a simple biometric screening by a healthcare professional to provide you with important statistics like blood pressure. You can take our form to your own physician and have it filled out as part of your routine physical exam, and submit it to Quest Diagnostics, or you can schedule the biometric screening at a Quest Diagnostics Patient Service Center near you. To see your own physician, schedule a routine physical exam and let them know when you make the appointment that you have a “biometric screening” form you need completed for your Wellness Incentive. Then simply bring the form along to your exam.

And, once we process your screening results, you can earn another $100 incentive reward which will be deposited in the Wellness Incentive Fund Account. Please allow 4 weeks for us to process the screening before your incentive is available. You can obtain this reward once per calendar year.*

The plan will reach out to you if you are identified through claims data as having high blood pressure and will provide you with a form for your provider to complete. On the form, your provider must document two (2) controlled blood pressure readings below 140/90 on separate visits during the current calendar year for you to earn the incentive. The completed form must be submitted to the Plan by December 1st of the calendar year. To earn the incentive, documentation should be mailed to: Aetna, C/O FEHB QM, 4400 NW Loop 410, Suite 101, San Antonio, TX 78229.

The Plan will reach out to you if you are identified through claims data as having diabetes and ask you to have your provider submit your A1c laboratory results. Your A1c laboratory results must be less than 8% during the calendar year for you to earn the incentive. Your A1c laboratory results must be submitted to the Plan by December 1st of the calendar year. If you are pregnant, your provider must submit documentation of a prenatal care visit during the first trimester. The documentation submitted must include a copy of the prenatal care medical record of Obstetric Panel testing from your attending provider for you to earn the incentive. The documentation must be submitted to the Plan by December 1st of the calendar year. It’s always good to start at the beginning: call your primary care or family physician today to schedule a physical exam (or a routine gynecological exam). This is an especially important step if you took the Biometric Screening at a Quest Diagnostics Center. Bring your new knowledge back to a doctor you want to work with, and ask them: What is the next healthy action I should take? Don’t forget: When you have your routine physical exam performed by an in-network provider or a provider outside the 50 states, the Plan pays for covered services at 100% Plan allowance – with no cost-sharing! 24-Hour Nurse Advice & Translation Lines 

FSBP offers two free 24-hour hotlines to assist our members in real time.

The Nurse Advice Line provides members with a live registered nurse to advise them about urgent issues and answer health-related questions. The Nurse Advice Line is 1-855-482-5750 or 1-705-834-6782, option 1. You can also engage a registered nurse in an online chat via My Online Services. Once you log on the site, select "Wellness Tools" and then "Nurse Advice Line." In addition, you have access to the AudioHealth Library within My Online Services. 

The Translation Line provides telephonic language translation services for our overseas members. This service provides direct translation for our members utilizing urgent or emergency services while outside of the U.S. The Translation Line is 1-855-482-5750 or 1-705-834-6782, option 2.

 

KidsHealth

KidsHealth is a complementary and comprehensive library of childhood health information featuring thousands of medically reviewed articles, animations, and age-appropriate news for three distinct audiences: parents, kids, and teens. Log on to My Online Services to access KidsHealth.

Use this online library to access the tools you need as a parent to teach your children how to lead healthier lives. The special kids' sections include fun and age-appropriate information, recipes, quizzes, and movies. The site includes information about dealing with emotions, stress, and school. Case Management Program

FSBP offers free and voluntary case management to all enrollees. Case management assists you, your dependents, and your healthcare providers to address acute, complex, and/or long-term medical needs. Case management services include nurse support; education about disease, illness, and injury; and proactive medical management that can help you lead a healthier life. Pre-Diabetic Alert Program

The Pre-Diabetic Alert Program focuses on providing members with the tools they need to live healthier lives when facing a chronic illness – diabetes – before it happens. We proactively contact those members who are at risk for developing diabetes to invite them to participate in this program. Looking for accurate health information from a trusted source? Use the Cleveland Clinic for free, trustworthy medical information. While at their site, you can sign up for free e-newsletters, physician-hosted online chats, or look up specific health topics.

Using the Cleveland Clinic website, including participating in the physician-hosted online chats, for health information is not part of the FEHB contract or premium, so you cannot file an FEHB disputed claim about it or the chats they hold.

In addition to our standard support for members going through cancer treatment, the TherapEase Cuisine program helps cancer patients answer the question “What should I be eating?” Provided in an online format by the Express Scripts Pharmacy (the Plan’s Pharmacy Benefit Manager), it follows the American Dietetic Association’s guidelines for cancer nutrition. More Wellness Programs

Our wellness programs include additional options outside of coaching-style healthy actions. As your health insurance plan, the FSBP works closely with many partners to provide you with professional services to keep you healthy. 

 

24-Hour Nurse Advice & Translation Lines 

FSBP offers two free 24-hour hotlines to assist our members in real time.

The Nurse Advice Line provides members with a live registered nurse to advise them about urgent issues and answer health-related questions. The Nurse Advice Line is 1-855-482-5750 or 1-705-834-6782, option 1. You can also engage a registered nurse in an online chat via My Online Services. Once you log on the site, select "Wellness Tools" and then "Nurse Advice Line." In addition, you have access to the AudioHealth Library within My Online Services. 

The Translation Line provides telephonic language translation services for our overseas members. This service provides direct translation for our members utilizing urgent or emergency services while outside of the U.S. The Translation Line is 1-855-482-5750 or 1-705-834-6782, option 2.

 

KidsHealth

KidsHealth is a complementary and comprehensive library of childhood health information featuring thousands of medically reviewed articles, animations, and age-appropriate news for three distinct audiences: parents, kids, and teens. Log on to My Online Services to access KidsHealth.

Use this online library to access the tools you need as a parent to teach your children how to lead healthier lives. The special kids' sections include fun and age-appropriate information, recipes, quizzes, and movies. The site includes information about dealing with emotions, stress, and school. Case Management Program

FSBP offers free and voluntary case management to all enrollees. Case management assists you, your dependents, and your healthcare providers to address acute, complex, and/or long-term medical needs. Case management services include nurse support; education about disease, illness, and injury; and proactive medical management that can help you lead a healthier life. Pre-Diabetic Alert Program

The Pre-Diabetic Alert Program focuses on providing members with the tools they need to live healthier lives when facing a chronic illness – diabetes – before it happens. We proactively contact those members who are at risk for developing diabetes to invite them to participate in this program. Cleveland Clinic 

Looking for accurate health information from a trusted source? Use the Cleveland Clinic for free, trustworthy medical information. While at their site, you can sign up for free e-newsletters, physician-hosted online chats, or look up specific health topics.

Using the Cleveland Clinic website, including participating in the physician-hosted online chats, for health information is not part of the FEHB contract or premium, so you cannot file an FEHB disputed claim about it or the chats they hold.  

myStrength™

Did you know that 1 in 4 people face mental health challenges, and about 70% do not receive the care and support they need?*

FSBP partners with myStrength to help our members access mental health support confidentially. myStrength is a unique online approach to putting mental wellness tools into the hands of FSBP members who need them. The program makes it easier for those facing some of the most common challenges – depression, anxiety, and substance abuse – to access real, evidence-based therapies, anywhere, anytime. 

All covered members ages 13 and older can sign up individually using the access code “FSBP.” New myStrength members first complete a brief Wellness Assessment and then are provided with modules specifically designed to help them deal with depression, anxiety, and substance abuse disorders.

What is the calendar year deductible?
  • For Self Only (401) enrollment, the deductible is $300 for in-network providers (including Guam) and providers outside the U.S., and $400 for out-of-network provides (including Guam).
  • For Self Plus One (403) enrollment, the deductible is $600 for in-network providers (including Guam) and providers outside the U.S., and $800 for out-of-network providers (including Guam).
  • Under a Self and Family (402) enrollment, the deductible is $600 for in-network providers (including Guam) and providers outside the U.S., and $800 for out-of-network providers (including Guam).

Once your deductible is satisfied, you have “Catastrophic Protection.” You can think of this as a ceiling on your out-of-pocket expenses. For those services charging coinsurance, rather than a copay, FSBP pays 100% of the Plan allowance for the remainder of the calendar year when payments exceed $5,000 for Self Only enrollment or $7,000 per person for Self Plus One or Self and Family for in-network providers and providers outside the United States and Guam.  For out-of-network providers on services charging coinsurance, the corresponding limits are $7,000 for Self Only and $9,000 for Self Plus One or Self and Family.  There are some expenses that do not fall under this rule; see your Plan Brochure, Section 4, Your Costs for Covered Services. 

What's the difference between My Online Services and the AFSPA Member Portal?

My Online Services (MOS) is an Aetna system that provides members with greater access to information and tools regarding their health. Every enrollee and dependents ages 14 and older have their own separate login credentials for this site.

Many FSBP programs are housed in your MOS account, including signing up for wellness incentives, seeing your Health Risk Assessment results, opting into electronic funds transfer (EFT), learning more about the quality and cost of your healthcare, and viewing your Explanations of Benefits (EOBs).

The AFSPA Member Portal is a custom system for AFSPA members. It can be used to upload claims, update addresses, and enroll in and manage certain Ancillary Insurance Programs. The Member Portal is only open to the AFSPA member, not dependents. We recommend having a Member Portal account in order to allow for the fastest processing of claims.
